Anti-infective drugs for regulatory registration", "description": "Abstract<p>The World Health Organization \uffe2\uff80\uff98Ending the neglect to attain the Sustainable Development Goals: A road map for neglected tropical diseases 2021\uffe2\uff80\uff932030\uffe2\uff80\uff99 outlines the targets for control and elimination of neglected tropical diseases (NTDs). New drugs are needed to achieve some of them. We are providing an overview of the pipeline for new anti-infective drugs for regulatory registration and steps to effective use for NTD control and elimination. Considering drugs approved for an NTD by at least one stringent regulatory authority: fexinidazole, included in WHO guidelines for Trypanosoma brucei gambiense African trypanosomiasis, is in development for Chagas disease. Moxidectin, registered in 2018 for treatment of individuals\uffe2\uff80\uff89\uffe2\uff89\uffa5\uffe2\uff80\uff8912\uffc2\uffa0years old with onchocerciasis, is undergoing studies to extend the indication to 4\uffe2\uff80\uff9311-year-old children and obtain additional data to inform WHO and endemic countries' decisions on moxidectin inclusion in guidelines and policies. Moxidectin is also being evaluated for other NTDs. Considering drugs in at least Phase 2 clinical development, a submission is being prepared for registration of acoziborole as an oral treatment for first and second stage T.b. gambiense African trypanosomiasis. Bedaquiline, registered for tuberculosis, is being evaluated for multibacillary leprosy. Phase 2 studies of emodepside and flubentylosin in O. volvulus-infected individuals are ongoing; studies for Trichuris trichuria and hookworm are planned. A trial of fosravuconazole in Madurella mycetomatis-infected patients is ongoing. JNJ-64281802 is undergoing Phase 2 trials for reducing dengue viral load. Studies are ongoing or planned to evaluate oxantel pamoate for onchocerciasis and soil-transmitted helminths, including Trichuris, and oxfendazole for onchocerciasis, Fasciola hepatica, Taenia solium cysticercosis, Echinococcus granulosus and soil-transmitted helminths, including Trichuris. Additional steps from first registration to effective use for NTD control and elimination include country registrations, possibly additional studies to inform WHO guidelines and country policies, and implementation research to address barriers to effective use of new drugs. Relative to the number of people suffering from NTDs, the pipeline is small. This work was performed in the frame of the French ANR BioSiSol project (ANR-14-CE01-0002) and is based on data from the RMQS program (French Soil Quality Monitoring Network). The French Soil Quality Monitoring Network (RMQS) is a national program for the assessment and long-term monitoring of the quality of French soils. This network is based on the monitoring of 2240 sites representative of French soils and their land use. These sites are spread over the whole French territory (metropolitan and overseas) along a systematic square grid of 16 km x 16 km cells. The network covers a broad spectrum of climatic, soil and land-use conditions (croplands, permanent grasslands, woodlands, orchards and vineyards, natural or scarcely anthropogenic land and urban parkland). The physical, chemical and biological properties of the soil are measured on each site. These soil analyses were carried out by the Soil Analysis Laboratory of INRAE (Arras, France). The spatial and temporal variability of soil properties are explained by biophysical variables, sources of contamination, history of land-use and management practices on each plot. The first sampling campaign in metropolitan France took place from 2000 to 2009 and the second campaign has begun in 2016. At each site, 25 core samples were taken by layer with an auger within a 20 m \u00d7 20 m plot and combined into a composite sample. Analyses used in this study only concern the surface layer (generally 0\u201330 cm layer) of samplings from the first campaign in metropolitan France. The analyses of total Si and available Si were made in 2016 on samples stored from the first sampling campaign. The dataset published contains all the raw data used in the statistical analysis in order to make them available for any further study. The table contains soil properties (total and available Si, particle size fraction, organic carbon content ...), observations about soil and land use, and spatial coordinates. We warn the user that coordinates published here are theoretical coordinates, the RMQS site can be located until 1 km around this point. This lack of contact with adult animals may hinder the rumen microbiological and physiological development. This study evaluates the effects of rearing newborn goat kids in contact with adult companions on the rumen development. Thirty-two newborn goat kids were randomly allocated to two experimental groups which were reared either in the absence (CTL) or in the presence of non-lactating adult goats (CMP) and weaned at 7 weeks of age. Blood and rumen samples were taken at 5, 7, and 9 weeks of age to evaluate blood metabolites and rumen microbial fermentation. Next-generation sequencing was carried out on rumen samples collected at 7 weeks of age. Results showed that CTL kids lacked rumen protozoa, whereas CMP kids had an abundant and complex protozoal community as well as higher methanogen abundance which positively correlated with the body weight and blood \uffce\uffb2-hydroxybutyrate as indicators of the physiological development. CMP kids also had a more diverse bacterial community (+132 ASVs) and a different structure of the bacterial and methanogen communities than CTL kids. The core rumen bacterial community in CMP animals had 53 more ASVs than that of CTL animals. Furthermore, the number of ASVs shared with the adult companions was over 4-fold higher in CMP kids than in CTL kids. Greater levels of early rumen colonizers Proteobacteria and Spirochaetes were found in CTL kids, while CMP kids had higher levels of Bacteroidetes and other less abundant taxa (Veillonellaceae, Cyanobacteria, and Selenomonas). These findings suggest that the presence of adult companions facilitated the rumen microbial development prior to weaning. This accelerated microbial development had no effect on the animal growth, but CMP animals presented higher rumen pH and butyrate (+45%) and ammonia concentrations than CTL kids, suggesting higher fibrolytic and proteolytic activities. CMP kids also had higher blood \uffce\uffb2-hydroxybutyrate (+79%) and lower blood glucose concentrations (-23%) at weaning, indicating an earlier metabolic development which could favor the transition from pre-ruminant to ruminant after the weaning process. The fungal:bacterial (F:B) ratio targets two major functional groups of organisms in soils and can improve our understanding of their importance and efficiency for soil functioning. To better decipher the variability of this ratio and rank the environmental parameters involved, we used the French Soil Quality Monitoring Network \u2013 one of the most extensive and a priori-free soil sampling surveys, based on a systematic 16 km x 16 km grid and including more than 2,100 samples. F:B ratios, measured by quantitative PCR targeting the 18S and 16S rDNA genes, turned out to be heterogenously distributed, and spatially structured in geographical patterns across France. These distribution patterns differed from bacterial or fungal densities taken separately, supporting the hypothesis that the F:B ratio is not the mere addition of each density, but rather results from the complex interactions of the two functional groups. The F:B ratios were mainly influenced by soil characteristics and land management. Among soil characteristics, the pH and to a lesser extent the organic carbon content and the C:N ratio were the main drivers. These results improved our understanding of soil microbial communities, and from an operational point of view, they suggested that the F:B ratio should be a useful new bioindicator of soil status. Six earthworm species (Lumbricus terrestris, Allolobophora chlorotica, Octolasion cyaneum, Octodrilus complanatus, Aporrectodea caliginosa meridionalis and Microscolex dubius) were incubated in soil cores with soil and alfalfa litter for 6 weeks, at 16 \u00b0C. The soil is a silty loam sampled in an agricultural plot in Estr\u00e9es-Mons, northern France. Two series of soil cores were performed, including controls (3 with litter and without earthworm, 3 without litter and without earthworm per series). At the end of the incubation period, the entire cores were scanned using a medical scanner for bioturbation measurements. All images were processed the same way to compute the number of macropores (set of connected voxels), the volume of burrows (sum of all the macropore volume), the vertical barycentre of the burrow system (the centre of mass of the burrow system when only the vertical dimension is taken into account), the diameter (assessed as the median of the diameter of all vertical pores with a circularity 0.8) and burrow system continuity (number of burrows whose vertical length was higher than 30% of the depth of the soil core). In cores with earthworms, the soil was sampled from three different compartments: surface casts, the drilosphere (i.e., soil surrounding burrows, up to 4 mm from the burrow walls), and surrounding bulk soil. In control cores without earthworms, the soil was sampled from three different depths: surface (0-2 cm from the soil surface), middle (8-12 cm) and bottom (18-22 cm). Biogeochemical measurements, performed on the different soil compartments, included : soil humidity, dissolved organic carbon (C), total C, total nitrogen (N), mineral N, organic N contents, C to N ratio, nitrate (NO3-) and ammonium (NH4+) contents at the time if sampling and after 8 days in incubation at 20\u00b0C, carbon dioxide (CO2) fluxes measured with Isotope-ratio mass spectrometry (IRMS) method after 46, 91, 136, and 181 hours of microcosm incubation at 20\u00b0C, CO2 fluxes measured with sodium hydroxide (NaOH) traps after 46, 94, 142, and 190 hours of microcosm incubation at 20\u00b0C. It can be investigated in two categories as primary and secondary neurulation. Primary neurulation takes place by formation of neural plate that starts with the proliferation of ectoderm, folding of the neural plate lateral edges, and raising of these folds and approaching and joining of them in the middle zone. Secondary neurulation, on the other hand, takes place in the caudal of the embryo and starts with the formation of medullar cord via grouping and densification of mesenchyme cells in the tail bud. Earlier investigations have usually covered the primary neurulation. In this study, however, process of secondary neurulation that forms through canalization of a massive group of cells is investigated. Secondary neurulation is studied in stage 15 of 55-hour chick embryo based on the Hamburger-Hamilton stages. After the removal of embryos from the eggs, they were dissected from their membranes and put into proper fixatives. Paraffin procedure was applied to a group of embryos for the purpose of light microscopy and histochemical studies. P.A.S. and Hematoksilen Eozin staining were done to some 2- micron sections taken from the paraffin blocks. In order to achieve a better resolution under the light microscopy, some chick embryos were prepared according to the electron microscopy sample preparation procedures, and epon blocks were prepared accordingly. Some 1 -micron sections taken from the epon blocks were stained by toulin blue. Finally, embryos were studied under the light microscopy, and their photos were taken. It was found out that secondary neurulation was formed via cavitation of the medullar cord that developed from the tail bud in the caudal of the chick embryo. Different from the primary neurulation, secondary neurulation in chick embryos has four stages: First, formation of a massive group of cells identified as medullar cord91 via joining of dorsal cells of the tail bud. Second, appearance of the edges of the area of the cells grouped and differentiation of the cells as central and peripheral cells. Third, cavitation between the central and peripheral cells. Fourth, joining of all lumens and formation of one central canal. Apoptotic cells were identified according to the histologic criteria in the secondary neural tube and tail mesenchyme formed during the time period investigated. It was thought that these apoptotic cells were used in the normal development and control of the cell number. Neural tube defects still stay an important health problem, even though the understanding of the neurulation has advanced in the last years. Neural tube closure defects in humans may give rise to neural tube defects known as spina bifida, vertebrate defects, and in the extreme cases paralysis. In addition, defects that come to existence during the secondary neurulation are observed in clinical cases as terminal miyelosistosel and thick filum terminale syndrome. Although, mechanisms in the molecular and cellular level are now understood better, understanding of the cell behavior is important in preventing the congenital malformations. Finally, It was declared that secondary neurulation is very important in the process of neural tube formation as neural tube formation is very important in the development of a normal human. New drugs are needed to achieve some of them. We are providing an overview of the pipeline for new anti-infective drugs for regulatory registration and steps to effective use for NTD control and elimination. Considering drugs approved for an NTD by at least one stringent regulatory authority: fexinidazole, included in WHO guidelines for Trypanosoma brucei gambiense African trypanosomiasis, is in development for Chagas disease. Moxidectin, registered in 2018 for treatment of individuals\uffe2\uff80\uff89\uffe2\uff89\uffa5\uffe2\uff80\uff8912\uffc2\uffa0years old with onchocerciasis, is undergoing studies to extend the indication to 4\uffe2\uff80\uff9311-year-old children and obtain additional data to inform WHO and endemic countries' decisions on moxidectin inclusion in guidelines and policies. Moxidectin is also being evaluated for other NTDs. Considering drugs in at least Phase 2 clinical development, a submission is being prepared for registration of acoziborole as an oral treatment for first and second stage T.b. gambiense African trypanosomiasis. Bedaquiline, registered for tuberculosis, is being evaluated for multibacillary leprosy. Phase 2 studies of emodepside and flubentylosin in O. volvulus-infected individuals are ongoing; studies for Trichuris trichuria and hookworm are planned. A trial of fosravuconazole in Madurella mycetomatis-infected patients is ongoing. JNJ-64281802 is undergoing Phase 2 trials for reducing dengue viral load. Studies are ongoing or planned to evaluate oxantel pamoate for onchocerciasis and soil-transmitted helminths, including Trichuris, and oxfendazole for onchocerciasis, Fasciola hepatica, Taenia solium cysticercosis, Echinococcus granulosus and soil-transmitted helminths, including Trichuris. Additional steps from first registration to effective use for NTD control and elimination include country registrations, possibly additional studies to inform WHO guidelines and country policies, and implementation research to address barriers to effective use of new drugs. Relative to the number of people suffering from NTDs, the pipeline is small.
